RATIONALE: New USP Chapter 797 standards for the preparation of allergy immunotherapy vials will change in December, 2023. These guidelines entail potential changes in facility and personnel qualifications, workflow, and laboratory documentation. USP 797 standards harmonize workflow and safe storage of antigens. However, changes require an ISO Class 5 Primary Engineering control Room (PEC) or an Allergenic Extracts Compounding Area (AECA).
METHODS: We hypothesized that the cost of adhering to these regulations may exceed $25,000 for a solo private practice suburban New York outpatient allergy office. Using a Public Service Enterprise Group (PSEG) grant application as a template, our architect calculated the cost, with Long Island, NY contractors, of converting a secondary waiting room into an AECA by 1) adding a non-load bearing door with sweep to preserve interior air quality, 2) removing the dropped ceiling and replacing with a sheetrocked ceiling, 3) painting all interior surfaces with a glossy, washable paint, 4) sealing the existing window, 5) cleaning all heating/ ventilation/air conditioning ductwork with ultraviolet-c light and installing MERV13 HEPA filters.
RESULTS: Predicted costs reached the $25,000 cap. These costs do not account for day-to-day operational 70% IPA wipes, staff certification at AAAAI courses, headcaps, gloves, masks, gowns and booties.
CONCLUSIONS: In conclusion, conversion costs to achieve USP Chapter 797 standards for mixing allergen vials meets the $25,000 estimate and likely is to exceed the cap based on operational costs.
We assisted Dr. Anthony Szema of Three Village Allergy & Asthma, South Setauket, NY, in securing a PSEG grant to construct a dedicated Allergenic Extract Compounding Area (AECA) in his South Setauket, multi-specialty practice. Collaborating with the practice we helped assess the financial requirements associated with constructing an AECA under USP 797 standards in a suburban healthcare practice.
